Fan issues with R520 Server

My Client is having some intermittent issues with the fans on his R520 server. I have included some of the reports from the unit , initial issues and reports after the re-boot.

Our question is do you think it may be a faulty fan sensor(s) or the fans themselves? Any input would be greatly appreciated !

I should also mention: 

Screenshots show:

PS1 power supply is showing ZERO Voltage

System Board Fans all show strange RPM values, which seems very odd as well.

Hello.

What is the status of the PS1 in OpenManage Server Administrator? The fans' speed is controlled by the iDRAC and depends on the ambient or outside air temperature within the server environment. Review the server environment, if fine, draining system flea power by disconnecting all power cables to the server for about 10 min. This will often lower the fans' RPM. Also ensure that the system BIOS and iDRAC firmware is up to date.
