Sounds like it may be a Firmware or Driver problem.  Are any of the drives offline?   I would suggest updating the PERC firmware and driver from support.dell.com

Dennis,
Thanks.  I will add that to the maintenance list.

I have a PE2900, and have received more or less the same error 2 times now.  This is directly from the SAS Storage Manager logs:

Controller ID:  0   Fatal firmware error:
    Line 1014 in ../../raid/verdeMain.c

Controller ID:  0   Fatal firmware error:
    Line 1029 in ../../raid/verdeMain.c

I've also received the following warning

Controller ID:  0   Unexpected sense:   PD
    =   1:255,   CDB   =    0x1c  0x01  0x01  0x08  0x00  0x00     ,   Sense   =    0x70  0x00  0x06  0x00  0x00  0x00  0x00  0x0a  0x00  0x00  0x00  0x00  0x29  0x00  0x00  0x00  0x00  0x00

I updated to the current PERC 5/i firmware:

Component Current Version
Package 5.2.1-0067
Firmware              1.03.40-0316
BIOS MT28-8
Ctrl-R 1.04-019A
MPT 00.10.61.00-IT

Drivers are the updated ones as well.  The only other thing that changed just shortly after I installed the firmware is I changed the cache policy from WT to WB.

Other then that, before this past round of updates, everything has been fine.

Joe, have you updated your server, have you received any similar problems?

Thanks
Josh

Josh,

I did apply the firmware update and have not had a problem since.

Sorry, that was months ago and I don't recall.  I am not at that site to check.  Check the Dell site for the latest available.

I used the latest firmware, which came out in November....and that's the firmware/driver I had issues with.

I talked with a dell support rep on Friday.  I've downgraded to the previous firmware/driver combo to see if I still have issues or not.  Unfortunately, the problem is random, and it took about a week between the two crashes.  So I won't know whether the older driver/firmware combo helps, or if I have a hardware issue.
