Backup to DLTVS160 - Hardware Failure

I have a PV-110T-DLTVS160 tape drive, connected to an Adaptec 39160/3960D Ultra 160 SCSI card, running in a Dell 600SC, with SBS 2003.
 
We have been using NTBackup to write to this drive since approx. July 2006, but the backups have been failing consistently in the past two weeks. We get the following errors in the logs:
 
NTBACKUP       Event ID 8019
NTBACKUP       Event ID 8001
QNTMDLT         Event ID 15 The device, \Device\Tape1, is not ready for access yet.
ADPU160M        Event ID 11 The driver detected a controller error on \Device\Scsi\adpu160m2.
 
The backup starts and a varying amount of data is backed up, but never more than about 20Mb.
 
I have since installed Backup Exec to see if this makes any difference, and this fails also, with the following errors:

Backup Exec   Event ID 57665
Storage device "DELL 2" reported an error on a request to write data to media. Error reported: The request could not be performed because of an I/O device error.

DLTTAPE-VRTS   EVENT ID 15
The device, \Device\Tape1, is not ready for access yet.

ADPU160M   EVENT ID 11
The driver detected a controller error on \Device\Scsi\adpu160m2.

I can control the drive from within Backup Exec, ie erase tape, eject tape. I can use NTBACKUP to write a backup to disk without any problem.

I have replaced the SCSI cable, but this made no difference.

I am thinking hardware failure, but i'm not sure whether it is the card or the tape drive. Any help would be much appreciated.

When was the last time you used a cleaning tape on the tape drive?
Have you run diagnostics on the tape drive?  This link might work, click here for the diagnostics

We have tried cleaning the drive several times in the last week, and had been cleaning it when the drive requested it prior to then.
 
I have run the diagnostics as suggested, and there are failures on the healthcheck. These are the results:
