It's an MXM III video card, but this system is old enough (and those are never produced in large quantity) that you'll likely be looking for a used/refurbished card.
One source: parts-people (link below) -- or EBay or similar.
The M15x can take up to the GTX 970M or the 4GB version of the 980M (which can be hard to find).
You will just need a modded Nvidia driver to run the card, which isn't too hard. Head over to the forum at NotebookReview as there are still a few guys over there who can walk you through the process.
