Historically, 4 com (serial) ports and two parallel ports were defined by address/IRQ.

Only if both Com1 and Com2 are realized in hardware. No reason to reserve a specific base address/IRQ if the hardware to use it is not installed.

When you install an internal modem, the modem creates the com port. If it is a hardware modem, it creates a true com port. If it is a software modem (WinModem, SoftModem) the it creates a virtual port. Neither of there will show up in Device Manager.

Com Ports on Modems are selectable. Most newer ones can be changed via the software. On the others you will need to change the com port via a jumper on the card. Set the com port on your modem to either 2 or 4. It is probably set currently to 1 or 3 - which are physically the same (as are 2 & 4). Also, did you go into the bios to make sure the com ports are enabled? The address is also selectable in the bios for each com port - so even if your modem is set for com port 2 if it is set to the wrong address it still won't work correctly. Check the documentation for your modem card to see which address it is using and set the com port address accordingly. Check the bios first, making sure it is enabled and then change the address and give it a try with each address before attempting to change the com port of the card.
