PCI bus failure
For some time I have had an intermittent problem with boot upon my Dimension 4100. Each time the fault codes have shown "Video Controller Failed to repond". I have reseated the video card a few times but the fault came back every 2 - 3 weeks.
Now I have a more serious problem. Every time the computer starts I am faced with a screen that tells me to "Press delete to continue". The fault codes on the rear show Yellow, Green, Yellow, Yellow which indicates a PCI bus failure has occured. When I press delete the computer boots up with no problems. I have removed all the PCI cards leaving only the video card plugged into the AGP slot and the fault is still the same. Any suggestions. Thanks

I had the same problem. It turned out to be an imminent hard drive failure. Since I was still under warranty, Dell replaced it. Try running the hard drive diagnostics which, in my case, showed a S.M.A.R.T. error, whatever that is. The instructions for running the test are here:

Nice troubleshooting work. I think you've hit the nail on the head.
My guess is that you've either an intermittent video card failure or AGP slot on the mb. I would call tech support (if still in warranty) to replace the card in an attempt to solve the problem - and also to be sure it's not the mb.
Very smart, by the way, pulling the PCI cards. Faulty AGP cards can corrupt PCI function in different ways.
