EqaulLogic MPIO not functioning on Windows Server 2012

We installed latest Windows updates on our Windows 2012 environment and afterward, EqualLogic MPIO on 4 out of 5 hosts (in the same cluster) are no longer functioning:

In iSCSI initiator, only 1 session is connecting to the volume and the MPIO tab shows nothing after the day of patching.

In ASM, multipath statistic is no avaiable sine the same time and all session shows "warning: iSCSI session configuration was not evaluated recently. Verify you are running the latest arry firmware version". Please note that 1 of 5 host works normally but other 4 don't. So I don't think it's related to firmware problem.

I've tried to upgrade HIT from 4.6 to 4.7 and uninstall+reboot+re-install. Still not working.

My environment:

Host: 5x Windows Server 2012 in a cluster
HIT: 4.6 (for troubleshooting, one of the affected host is now HIT 4.7)
EqualLogic firmware: 6.0.5

My EQL group consists of one PS6100 and one PS4000. I try ping from all interfaces already. It's ok. Support case is open both locally and support web site for some days, but still no useful feedback yet (I was told to reboot and etc..)
