1. Change/modify the IQN on the array Access tab to be updated for the new IQN which is seen in the MS imitator on the Configuration tab. They have to be identical.
2: On the server the MS initator must have ALL of its iscsi volumes logged out of and have the Favorites tab cleared so that it is empty.
All volumes can then be logged back in.
If using the EqualLogic HIT MPIO DSM, connect and ensure you select the checkbox for MPIO and "Reconnect to this volume on server restart". This will create a new Favorite target entry and a new registry entry on the server, and the HIT should create the additional connections automatically.
If using the Server 2012 native DSM MPIO you need to login again, select the MPIO check box, to connect to the volume the fist time, then manually login each interface used for MPIO (the first login will create the favorite again).

Yes we use EqualLogic HIT MPIO. So:
1. Connect to EqualLogic HIT MPIO and ensure that is selected the checkbox for MPIO and "Reconnect to this volume on server restart".
2. Change/modify the IQN on the array Access tab.
3. On the server the MS initiator log out all volumes.
4. Have the Favorites tab cleared so that it is empty.
5. Log back in volume.

1. Change/modify the IQN on array "Access" Tab
2. Disconnect from the array on the host (you will lose connection so plan accordingly)
3. Clear Favorites Tab in the iSCSI initiator
4. Refresh targets tab to ensure you can see the targets, if not, double check step 1
5. Login/Connect to the target, selecting the enable MPIO
6. On the array, you should see the host connection
7. @3 Minutes later you should start seeing the other interfaces making the MPIO connection to the target
