Dimension Weird Issue

I am working on a Dell Dimension 3100 and am having some flaky issues with it.
 
When I connect the display through a KVM switch it does not display anything.  The screen will flicker but nothing.  The KVM cable is fine.  No bend pins, working on other PCs.  If I connect a monitor directly to the PC I am able to get a display.
 
2nd Issue.  USB ports.  The two front connectors do not work.  And 2 out of the 4 in the back do not work.  At first I though it was an issue with the motherboard so with the system still being under warranty I had the board replaced.  When the new board was installed.  Same issues again.  Display and USB.
Does anyone have any thoughts or ideals about this?

Thanks, but it would be tough to install with no keyboard or mouse. 
 
One thing I forgot to mention.  After the motherboard was replaced.  None of the USB ports work in Windows XP.  The keyboard still works in the Bios but as soon as Windows begins to load.  No response.  Even the light for the CAP key goes out.
 
Any thoughts?

At this stage do you have any access at all (keyboard or mouse?). no ps2 ports to use a different mouse?
 
Try unpugging all usb plugs, mouse, keyboard and anything else usb ALL of them. Shut down comp and actually unplug from the wall socket. Leave 15 mins or so then reboot. Once system is up try plugging mouse in, use a back one. see if it is found. If it is or if you can use a ps2 mouse we need to clean up all the  usb controllers.
 
If you get access, try the following steps.  click start, then right click my computer and choose properties. Choose the advanced tab then

Click on ‘environment variables’ down at the bottom of the page.

Choose new in the SYSTEM variables box.  (if you choose User box the changes are only made to that particular user)._

A new Box will open..

 

In Variable name type: DEVMGR_SHOW_NONPRESENT_DEVICES

In Variable value type: 1

 

Then click ok, ok and ok..

 

 

Now open up device manager. Right click my comp,properties,hardware,device manager.  Click view and show hidden devices.

 

This will now show the usual hidden devices and the Ghosted ones that we want to remove. The Ghosted ones are recognisable as they are shadowed/greyed out.

Expand your USB controllers, there could be quite a few ghosted entries showing, uninstall each of these are they are not present on the computer. If you have a listing named ‘other devices’ remove all ghosted entries there as well.

Also expand ‘keyboards’ and the one for ‘mice’ there should be at least one (if not more) ghosted entries of the devices you are having trouble with. Uninstall them too.

 

Lastly unplug ALL USB devices on your computer and reboot. TWICE.. This will reset all USB;’s.  Hopefully now when computer has restarted your USB will be recognised. (as new hardware) and all should be good.
