AC Adaptor Failed... AGAIN

Several months ago my AC adaptor for my Inspiron notebook failed... (no green light). I promptly ordered a replacement. They are not cheap! Now within a couple of months it has failed again.
 
Has anyone else had this experience of poor reliability? Also, I have tended to leave ot plugged in when the lap-top is not attached. Does anyone know whether this could be the cause of the failure?
 
Perhaps it is just poor luck, but I would like know if I am doing anything wrong before reordering.

oops, sorry for the lack of specificity

I have an Inspiron 600m, 1.4Ghz with 384MB DDR, 30GB ultra ATA harddrive, running windows XP, SP2.  The adaptor is a PA-12 Model AA22850, 65W manufactured by Emerson Japan, LTD

I am aware that some adaptors fail, but I was hoping for more insight as to why this one failed so quickly, before I send another $40 to Dell. Is there something I could be doing, or something wrong with the laptop that would cause these failures? Any help would be appreciated.

What model notebook and adapter. Have others had AC adapters fail, YES. Is it the same model adapter as yours?????????????????????  Was it used on the same model notebook as yours????????????

suppose it couldn't hurt to put a good surge protector or even UPS in front of it...
