I have the same issue on 2 new S4048's taken right out of the box....now im more worried because i have 3 more in a box and this is going to cause issues.
Is there any solution to solve this problem? I just bought two S4048-ON and four S3124 switches, I can logon 3124 via console, but S4048-ON can not. I can not do anything with S4048 now. Need I reboot the 4048? Oh, MY GOD!!!
I have this same issue on the S4128t-ON. I have the stated settings, but I am unable to connect unless I open the connection and reboot the switch. Otherwise I get a blank terminal.
I have to assume this is not by design, so what gives?
I was having the same issue but using advice here I can get in, had to use 115200 the problem I am having now is with PuTTy and Windows 10 the console CLI seems to not recognize key inputs from my laptop. Will not recognize Del, Esc or F2, so I can't stop the autoboot process, when it comes to BMP attempts I go to special commands in PuTTY and hit break, it gives me a menu. When pressing A to Abort it will not work? Anyone else had this problem.

I am using the cable that the switch came with so don't think pin outs are the using.
Also this cable works with the n2048 switches

We have found that by configuring the Putty or SecureCRT with the recommended Console settings, and powering up the switch, the console then works.
The trouble will be when trying to access these switches once the switch is operational.

If anyone else hits this page, the 4048T-ON we had delivered has the ports reversed per the documentation.
The Console Port is the top port per the logo on the switch. You want to plug your cable into the I0I0I0 logo which was the top port for us.

Are you running latest firmware? (there's a bug in an earlier version where console/ssh access might fail)
