N1548p POE Issues/Failure

I've been having POE issues across multiple 1548p switches. Firmware version 6.5.1.6. I've had 2 switches that had total POE failures. The switch remains online, but all POE ports report a state of "Test Fail" when running the show power inline command. I was sent a replacement for these switches. before the replacement was sent out, I upgraded to 6.5.2.9. This didn't fix the issue.

Today I had another POE issue on a 1548p, all APs connected to the switch (Aruba 215) were only receiving around 4400-4900 mW power. Working APs are normally closer to 7500mW. Rebooting the switch caused power levels to return to normal

I've done some research, but I'm unable to find any documentation of issues like this. Is there any other troubleshooting I can do to determine the root cause of these POE issues?
