DELL 8200 Blue Screen when dial up using internal modem! Then restart automatically

Hi,
 
Recently when I use the internal modem, blue screen will occur and my notebook will restart automatically.
Error on the blue screen 'sTOP: 0X000001E (0XC000005,.....    Adress BB583FE4 base at BB57B000, Datestamp 3D32BE87-HSFHWICH.SYS'
My notebook spec:
-Dell Inspiron 8200 downgraded from Windows XP Pro to Win2K Pro (SP4)
-I've update all the components in www.windowsupdate.com
-Tried reinstall the modem driver
-Tried reinstall WIndows 2k Pro
-Modem by Actiontec MD56oRD V92 MDC Modem (driver v 5.3.21)
If I dial using an external USR modem it's working fine.
1 out of 10 time I managed to dial using my internal modem, so I guess it's not the modem faulty.

Ok, now boot to the Windows Safe Mode and get into the Device Manager, delete all modem entries, reboot and let the modem be found again then make sure it was the R50360 driver it reloaded.  See if that will do it.

I've deleted the modem is safe mode and restart the computer in 'Normal' mode, it does not detect the correct modem, it just shows PCI modem with and exclaimination mark on Device Manager. I've tried all 3 versions of drivers for the modem from the DELL website.

Even tried formatting it to Win XP pro and try again, but no luck :(
