Honestly, it sounds like the card itself needs to be replaced. I recommend calling tech support and explaining the problem and the troubleshooting steps you've already done, and their results. Assuming you are still in warranty it should be a simple matter at that point to do set up the replacement since the only thing you haven't done is reinstall Windows (and I really don't think that will help, based on what you describe).
