Problem with PowerEdge 800 Server Video

We have this Problem with PowerEdge 800.

Lately when we turn on the monitor it does not show the desktop and only shows "Going to Sleep Mode"

Sometimes it works and sometimes it does not (even if we move mouse or click a key)

I am assuming it is Video card issue. Pls correct me if I am wrong.

If it is a video card issue, which is integrated in the mother board,

1. How do I replace it?

2. With which one? Pls. give specific part info. (if it could be generic also)?

Just for FYI - the PE800 can take many video cards that fit the available slots and can be powered.
The long PCI-X slot could take an old expensive video card.  Matrox for example.

The PCI slot can use an old style readily available PCI video card.  Nvidia 6200 Is my backup card.
Much newer cards can be found.

I recommend using the PCI-Ex1 very short slot for a video card.  Just about any PCI-Ex1 video card is an improvement to the embedded Intel E7221 chipset VGA-only video.

I have installed a Dell OEM by AMD HD8490 PCI-Ex16 video card via a PCI-E x16 to x1 adapter.  It displays online video just fine, with HDMI or DVI outs.  This can output HD audio.
However for newer GCN technology in an AMD card,. at least an HD8570 card is required. 
Full sized cards fit, they just have an extra port is the only difference.

Note: the PE800 has NO ONBOARD AUDIO.  I bought an audio card but the HDMI output can include audio too.

The video card has revived my ancient Pentium-4 660 - 4GB DDR2 ECC computer.
