Raid Problems
I logged in to one of the servers I take care of and got these messages
1st message was this
a block on the physical disk has been punctured by the controller
2nd was this after I pushed okay for the 1st message
there was an unrecoverable disk media error during the rebuild or recovery operation
System is a Dell Server PE 1900
raid array of 5
Is this serious???
How do i fix this ?

A puncture can also be called a double fault ... meaning that there is a bad data location on the same location of two disks. In a RAID 5, if one disk fails, the data is all accessible because the RAID 5 parity can calculate what is missing. A puncture/double-fault means that both the data and the parity locations are corrupt. This indicates data loss (although there may or may not be any crucial data stored there), so there is no way to fix it.

The safest choice is to delete the raid set and recreate it letting the controller do a (background) initialization. This way any media errors will be found and sectors remapped (if this even was part of the issue), and then you can restore your data.
