powermt config does not do it ? I have a seen that on a couple of my RHEL 6 boxes. During regular monthly patching window they would get rebooted and that paths would show up correctly.
same thing happens with other paths or only on this path(0A3B).if same thing not happens with other path please create a new meta device and assign on the same FA port and check now ,is it same .if no please take backup from volume 0A3B and restore on new vol.
Sol 3:
if it doesn't work please check power path version and os version and patch.check with power path comparability matrix from powerlink.if possible Please schedule a reboot for this host but b4 take backup .

You might try Powerpath does not see HW path and I/O path It seems to be very similar system to yours.

sol 1:
can you please try with following command
powermt dev=so
powermt config
powermt display dev=all
powermt save
sol 2:
