Autoloader Errors in BE10.1/124T-LTO2

Hello all, I've been having some problems with my Autoloader 124T-LTO2 unit with BackupExec 10.1.

Configuration:

Library: Autoloader 124T, Version: 31.0
Drive: LTO-2, Version: 1826
Drivers: Latest Tape and Medium drivers

Backup Server: Dell PowerEdge 1850
OS: Windows 2003 Server Standard R2 SP1
SCSI Card: Dell PERC 4e/Si
Drivers: Latest PRRC 4e/Si
Software Symantec BackupExec, Version: 10.1, Rev: 5629, Updates: SP1 Hotfix 26 Hotfix 27 Hotfix 29

Problem:

This configuration has been working well since we got it around 2 months ago. In the last 2 weeks I've been having troubble with media being loaded to the tape drive. This appears to be happening mostly on operations such as Library Invintories and Tape Label Medias. On operations such as this, the job failes and the LTO-2 drive goes offline. I have found a power-cycle of the Autoloader usually fixes the problem however the frequency and time to resolve (now takes more 'fiddeling' to get the drive back online) of this issue has been increasing. When the drive goes offline in BE, the following is errors are displayed on the Autoloader:

Flags[1-64]:0000000000002200
TA[51]-TapeDirInvalid
TA[55]-LoadingFailure

These errors don't give me a warm fuzzy feeling (especially the TapeDirInvalid). Has anyone seen this before? Any guidance would be greatly appreciated as I've updated everything to latest versions and I'm kinda at a loss.
