Are you having the same ME4012 that has the same error with the same SFP module part number?

Could you provide more details? 

What is the controller firmware version? What is the part number of the SFP you have, what is the error and part number shown in the port information? Do you have other SFP to test with?

The hardware is ME4012

The firmware controllers are GT280R009-02-dellemc

   we have another sfp, and we are going to test with our new ME. this SFP is older and could works. Ill let you know ASAP.

Test it with you available SFP to check the issue. Also, what are your other end device is? - Switch or Server? What protocol are they? If it's a switch, is it a FC or iSCSI switch? If it's a direct server connection, what is the HBA?

Is directly connected to a Server  R740xd.

Have you tested with another SFP and what is the HBA used for the connection at the server.
