This could be a drive issue, a backplane issue, a raid controller issue, or if it's in an MD1000 (that's daisy chained off of the MD3000), it could be a cable or EMM issue.
I recommend you call PowerVault support and troubleshoot this with their assistance.

This issue is only on slot6 HDD of MD3000.FYI the hdd is new .

If it's a SATA drive, it could be missing the SATA interposer board.
