Hard drive problems
I have a Poweredge 420 server with a raid 5 controller with 4 600G drives running windows 2016 standard server. I looked at open manage the other day and saw that drive 0:1:0 wasn't even showing in the list and drives 0:1:1 and 0:1:2 had failure predicted. I didn't realize there was four drive until I went to change one of them. The first drive I changed was 0:1:0. I waited for it to rebuild and when it was done it said failure predicted but it was online. I changed out 0:1:1 and it is currently rebuilding but now I have an error stating "The Virtual Disk has bad blocks. For more details, see the Virtual Disk Bad Block Management section in the Online Help". I am not sure what to do here. This is my Primary Domain controller.

I would be looking at replacing the predicted fail hard drives, then recreating the array and deploying Windows from a known good backup. With the virtual disk rebuilding, you're only going to see more bad blocks populate. The virtual disk may or may not stay online, but I wouldn't treat it as viable until redeploying from backup.
If you want to attach some screenshots from OpenManage, or export a controller log for review, I'm happy to look at them, but I don;t expect my recommendation to change.
We have a white paper on double fault conditions, and while lengthy, should help explain why I'm recommending a redeployment.
