Surachet, I moved your thread to Symmetrix. I would suggest opening up a service request to have the Symmetrix checked out. It's hard to say from a few screen shots if this is the root cause of Oracle corruption and deeper investigation of the Symmetrix should be done.

What does the messages or event log show?

what is the configuration of that particular LUN?

which OS and powerpath version?

I've see corrupted Oracle on AIX because of power path pseudo devices were not matching their major/minor numbers
