Hi cincyfan.
I was on the site responding to another log and I saw your post. I don't have the time to research your log line by line right now, but I do have some suggestions for you that should improve your situation.
First, put hijack this in its own folder such as c:\hjt so its backups will be safe if you clean temporary files.
Run Hijackthis with browser windows closed and allow it to fix:
I know based on other reviews that these r lines and host file lines are bad:

I have not researched these o16's, but they are the ones that look suspicious to me.
Unless they are things that you know to be safe, go ahead and fix them:

Reboot.
Run an online virus scan.
Trendmicro, Pandasoft, and RAV are three that I see experts recommending frequently.
You can find some links here:
http://www.newbie.org/firstaid/healthcheck.html
bleepingcomputer.com has tutorials for spybot, adaware, and cwshredder. Go check those out and then get each of those programs and run them.
cwshredder
spybot
and adaware.
Then reboot and run a new hijackthis log. You can post it here on the chance I can have some time next week, but you would probably be better off posting it on another site as well. I hope those steps will have made a significant reduction in the number of problems you're dealing with.
I would be interesting in knowing how things go for you.
Regards.
