PV120T DLT1 keeps going "OffLine"

We have a PV120T that has started showing up as "off line" in Backup Exec 9.0.4454.  Simultaneously, we have started getting inundated with Parity Error messages in the System Log (Win2k SP4)  Event ID 5 from the SCSI driver. 

The drive was just installed on this machine at the end of March, and it worked perfectly for two weeks.

Nothing has been changed on the tape drive.  Here's the part that makes me cringe to ask... It's not connected to a Dell Server.  It *is* connected to a HVD SCSI card.  At first I thought the cable might have been damaged physically subsequent to the install.  I had the person on site try two different cables and we get the same result.

The SCSI drivers are up to date.  We are using the Veritas Driver for the tape drive and the Dell Driver for the 'media changer'

This machine is 1000 miles from me, so I have no other device I can try on the SCSI connection, nor another machine that the tape drive can be connected to.

I'm trying to figure out if the drive/autoloader is failing or if the SCSI bus is failing.
