Need help to fix video screen random freezing + sometimes audio freezing as well.

Need help fixing screen freezing which happens at random intervals e.g. sometimes around every 20mins, sometimes even faster. Sometimes the audio freezes as well. Tried troubleshooting tools and some online research but wasn't helpful. Would be helpful if someone could help me out asap
