By any chance have you switched from IBM fibre channel interface to EMC fibre channel interface?

i would open an urgent ticket and have support run a health check on your DMX.

It could be a reservation issue on AIX. Whats the policy set to? Is this a device that is allocated to an HA cluster or a pair of VIOSs?

Are you getting the errorss for only this disk? Is SC3 bit set on Symmetrix for AIX host?

You can check on the device using command

symmaskdb list database -wwn xxxxxxxxx -v

You can check the setting of the director which the host is connected to first:

symcfg list -dir xx(for ex 7a) -p x(for ex 0) -v

Look for the below line in SCSI flags:

SCSI_3(SC3) : Enabled

If not;

Check the VCM config with:

symmaskdb list database -dir xx-p x -v

Look for:
Heterogeneous Host : --> This may be IBM_AIX
Port Flag Overrides : Yes
Enabled : SCSI_3(SC3) ---> Or this may be enabled.
Disabled : N/A
