Hard drive failure

Dell Vostro 15 3568

OS: Ubuntu 20.04.3 LTS

SSD - 256GB

Kernal 5.11.0.27
Secute boot Disabled
Legacy
Ubuntu only

Facing hard drive failure issue occasionally. Below mentioned problems are facing (step by step):

#1: WiFi disappears suddenly while working (showing driver not found).

#2: System freezes after some time of WiFi issue.

#3: On 'shutdown' taking too much time in "Waiting for process: wpa_supplicant".

#4: When reboots showing disk failure in 'swap' partition, and asking to run fsck.

#5: After running 'fsck' working fine for some time (15 - 60 mnts). After that again these steps are occurring.

Note : While wake up from sleep mode, also facing this problem.

Previously this issue totally corrupted my 1TB HDD and whole data. Then I changed HDD to new SSD. After 2 months these problems are again happening.

Kindly help me to trouble shoot this issue.
