Attempt to deploy CSM proxy on Azure fails to proceeed as it fails to save on proxy deployment page with message "Proxy Configuration details are missing"

Error
The proxy config page is properly updated.

Clicking on save throws proxy config missing error.

Solution: 

The user when at  azure proxy configuration page, needs to select the 'Add' button before saving the proxy configuration.
