Considering the fact that it works properly on the external display, it looks more like an issue with the LCD screen and you may need to have it replaced. If it were an issue with the video card, you would also experience video issues or distortions when playing games or watching videos on the external display.
The LCD is bad. My monitor looked exactly like yours when it went. I have one of the first M17x Dell came out with and I had this same issue along with so many other issues. Not only did I have issues with the hard drive, memory, and LCD screen but also with tech service. Dell's technicians had complete control of my computer remotely for 100,s of hours literally screwing it up to the point that I sent it back to Dell. When I got it back a week later a note inside said it was thoroughly tested and found to be in good order. It was the same as when I sent it back to Dell they fixed nothing on it. They then sent a technician to my house 3 different times which in the end he recommended they replace my computer because after he replaced the motherboard and hard drive and memory the computer would turn on and go black. Dell did replace it which took 2 months before I got it. But in the end I had all the same problems with the replacement computer. Again everything that could be replaced in this second Alienware was replaced, including the motherboard. They should of just sent me a third computer. But after everything was replaced the second time in the second computer (knock on wood) it has actually been running decent other than I still get blue screened quite often if doing any gaming. I would never buy another computer from Dell even if my life depended on it.

This was all replaced on the computer they gave me to replace my original Alienware that had multiple issues that they could not fix. All I can say is good luck to you. Personally, I told Dell I would never, ever buy another computer from them. Sorry I can't help you more.

There is a twisted update to my problem:
After 8 weeks of flickering the screen fixed itself. It was OK for 3 days. Then it went bad for 2 weeks.
Next it worked fine for another 6 weeks. All without touching anything. Just switching it on / off every couple of days.
An hour ago I picked the M17x up, and the screen went bad again.
Any ideas where to look ?
