I'm gonna ask five more questions before offering any advice here.
1) What is the GPU (ex GTX 780m)
2) What is the CPU usage while in game?
3) When in GPU-Z is a Perf-Cap reason listed under Sensors (You might have to add PerfCap to the page)
4) Does the GPU usage fluctuate a lot in game as seen from GPU-Z like going to 10 to 50 to 25 to 100, etc (in a short amount of time).
5) Did you update the graphics driver recently and did you notice the issue soon after updating it?
In fact, if you could it might help me if you just use the snipping tool that comes with Windows to take a snip of the sensor tab of GPU-Z and upload it to here.

The graphics card (GPU) might not be overheating, but what about the processor (CPU)? If the CPU is overheating then that could be the issue.

When its throttling what is perfcap reason in GPU-Z? (examples are like v-res, tmp, etc). Also, what is the power usage % when it is throttling?

No, the cpu is good no overheating issue at all, the clock is stable and temp to. It just the gpu that the clock is unstable because the throtling

Only when temp reach 67° the gpu clock drop to 1xx mhz to maintaim the temp
