How to mail Dell support

My effort to e-mail Dell support at

http://support.ap.dell.com/support/supportrequests/apj/create.aspx?c=in&cs=indhs1&l=en&s=gen&request=true

is so far unsuccessful. The message on the page tells

"After you have submitted the service tag, please continue to fill up the template below with your issue description and the contact information."

 I  do submit my Service Tag, but cannot see any template to advance further. Does it work ?
