Do you have an "Intel(R) 537EP V9x DF PCI Modem"? This this modem is a data/fax only modem. The "Intel(R) 537EP V9x DFV PCI Modem" is a full telephony modem (note the "V" in "DFV") and appear as a Voice modem to TAPI.
Yes, it's a DF, rather than a DFV. It seemed that even so, it was able to provide some kind of voice handling, but from what you've just shown, it seems it's not supposed to, and the easiest solution is to give up and buy a compatible modem.
