Not sure if optiplex 3060 allows you to use both integrated and discrete video ports at same time.

3060 system setup/bios video screen option:

Option Description
Primary Display
Allows you to select the primary display when multiple controllers are available in the system.
• Auto (default)
• Intel HD Graphics
NOTE: If you do not select Auto, the on-board graphics device will be present and enabled.

you also need to make sure your optiplex 3060 psu has enough power for the added discrete video card.  What is spec of your new video card?

If you have three video connectors on the new video card, and enough power for the card, you can support three display monitors all connected to the video card. The integrated video ports may be automatically disabled when bios detects the presence of video card.

Hi @redxps630 , thanks for responding.  You're misunderstanding - I'm not using a video card, I'm using the additional VIDEO header on the motherboard along with the Dell brand DP Port which installs into Option Slot 2, right beside the native video ports.

My expectation is that this IS using the integrated video, not a PCIe video channel.

Any further ideas?

Sorry I misunderstood your issue. Re: Dell brand DP Port which installs into Option Slot 2, I think you probably meant this one.  Your understanding is correct that this is an additional video port from motherboard integrated gpu.  
do you have 3060 MT or SFF?  MT has three PCIex1 slot plus PCIex16 slot.  SFF has one each.  Have you tried all available slots?

Yes, @redxps630 , that's the accessory I've added.  Again, it doesn't plug into any of the PCIe slots, it plugs directly into a dedicated port on the motherboard.  It's the 3060 MT that I'm using.

Sorry I misunderstood the DP port motherboard connector.  I think you meant it plugs into #5 video port header.  This DP port is a Dell proprietary mini adapter and cable assembly.  It may be the 3060 bios did not detect this added adapter after you connect it.  Try clear CMOs settings by jumper reset on motherboard.  Since this model is relatively recent, the CMOs battery is presumably still good.

The optional DP port is not a discrete video card.  It has no gpu.  It is merely a Dell proprietary adapter to create one more integrated video port on rear panel.

No, it wasn’t.  I gave up on my hope of using three displays on my 3060.

Optiplex 3060 Spec Sheet.pdf 

I know it's not cheap, but here's one of the GPU's listed on the spec sheet.  It has 4 HDMI ports:  VisionTek AMD Radeon RX 550 Graphic Card - 4 GB GDDR5 - Full-Height https://a.co/d/bXaKgyD

The NVIDIA GeForce® GT 730, 2GB is also listed, but its 3 display ports are all different - 1x DVI, 1x HDMI 1.4a, 1x VGA
