From the logs it appears a lot of what you are seeing could be attributed to the server being out of date on firmware. Would you confirm the versions of the BIOS, iDrac, and definitely the raid controller installed? Depending on how far back they are we may need to walk them up to current.
Let me know the versions and we can go from there.
Thank you. You stated that when the drive is rebuilding then it is stable, does the issue appear after the drive finishes the rebuild? Is it the same drive that is repeatedly the one that fails or goes foreign?

After first crash we updated all firmware and drivers. And now we have :
