PERC 3 BIOS Update

I am an administrator taking care of  12 Dell 6400 Poweredge servers with PERC 3 raid controllers.  Recently we have a hard drive failure in a raid 5 array, we replaced the drive and it began to rebuild OK, it finished the rebuild in about an hour and as soon as it finished it reported a failure. this process was tried again with the same results, we discovered that the drive was not putting itself back online after it rebuilt, we had to do this manually, as soon as we did that , the array came back and everthing was fine.  The problem is that this is an Exchange server and during the rebuild something happened to E, this is where the I.S. is.  We had to restore the IS from backup resulting in lost emails

I called Dell support to see if they knew why the drive did not come back online, they told me that there is a new PERC 3 QC (LSI formally AMI) BIOS update and NT 4.0 driver update. And that Dell is sending this out to be done on this hardware by Dell technicians.  Must be important.  I cannot find any information about this on the Dell site

Can someone provide me with information about this issue
