Your firmware is out of date on the Perc 5/i. Update the firmware on the controller to 5.2.2, download here: http://bit.ly/SiiQh2, Extract files to a bootable USB key and run the update.bat file, restart the server and boot to the Ctrl+R bios, select the foreign tab, and clear the foreign configuration from the array. Then restart the server.
