Thanks for the details. This is a 2 slot chassis only. Is the riser you have on the server is came with the server or you installed it later?

Second rise part# for this configs are. You can use one of these. First one support X16 slot and second one support X8 slot.

CY3R8 -  ASSY,CRD,EXP,RSR,CNTR X16,R630
JR5D2 -  ASSY,CRD,EXP,RSR,CNTR X8,R630

You are mostly seeing the issue as one of the riser is mossing. Did you have latest iDRAC and BIOS FW installed on the server?

Riser combination will depend on what chassis you have. Can you confirm whether you have 2 Riser chassis or three riser chassis. You can refer below link for more details

6R1H1 is a supported riser for R630. Once you confirm the chassis and part# of current riser I can confirm whether this riser is supported on your server or not?

The chassis is the 2 riser version.

I found the following numbers/PN on the riser card:

T2587401 Rev A01.

SN: R947CP0883 R

Part number will be 5 digit like "6R1H1". Can you share a picture of the Riser and which slot (17,18 or 21) it is connected to the motherboard from below link.

It is connected to slot 21 (however on my system board it's slot 29 accordingly to the chassis drawing but that perhaps do not matter here...). Pictures of said riser:

Please share the part# as mentioned by Young. It is available on the second picture with white square sticker  with QR code. 

Looking at the picture this riser looks like a LP riser (Part # KKVN7 or NG4V5). If this is correct then you have 3 PCIe slot chassis and you need riser 1 which will be having 2 LP slot. Part number for this riser is YNF4C, 999FX, DJG93 or 6GG48. Once you confirm the part# for you current riser we can conclude on this.

I'm somewhat confused about the chassi as I thought I had the 2 PCIe one but my looks not really like the 2 from the manual upon closer inspection:

Right, as Shine say we see only 2 slots, so you can't use 3 riser. It is your call but I also see that it looks a bit disfigured as well so maybe getting a new one might not be a bad idea.
