Can you find out which device is showing fatal error? For this you can login to iDRAC and check Hardware Inventory. If you expand various PCI devices on hardware you will be able to find bus, device and function number and able to identify which device having bus 128 device 3 function 2. Once you identify this you can try updating the firmware on it to see whether that can resolve the issue. You can also try powering off the server, remove power cable and insert power cable and power on the server after 20 to 30 seconds.

Unfortunately I'm not able to find which PCI device it is. I've gone through each one of them in Hardware Inventory and none is on bus 128...

 

I've exported the full SupportAssist logs and I'm happily willing to share them if this can help to find where the issue is coming from !

As you are not able to get exact device with bus number 128, I would suggest remove all PCIe cards from the server and try to boot and see whether the issue occurs. If not you can add cards one by one and check the behavior.
