Sounds like that particular nasty messed up your .exe file extensions, but neither Spyware Doctor nor Norton actually repaired it.
Given that it's malware, I'd get an expert opinion to make sure you really are clean. If you can download and run HijackThis, try the Malware Removal Board below. Be sure to read "How to Post" at the top of the board first:
