It is odd that you must be in safe mode to update anything, but I do know that Microsoft ended support for Windows XP Service Pack 2 in the summer of 2010. You will not be able to get updates for Windows XP unless you have updated to Service Pack 3.
Updating via Microsoft Update to Service Pack 3 can be troublesome for certain configurations. I generally download the package for IT Professionals to a USB memory stick or to a CD so that I can totally disable the anti-virus and perform the update off-line. You can find that update here: Windows XP Service Pack 3 Update. It is around 300 MB in size so be patient while you perform the download.
What happens if you try to use Windows or Microsoft Update in normal mode? A fresh installation of Windows often needs the Microsoft update to install the most recent version of the Windows installer. Many other updates won't work if the built-in installer application is out of date.

What security software are you using, it could be a firewall blocking Windows Update?
