Spoke too soon and just fixed it. While all my drivers and everything else was up-to-date, I downloaded this from the Dell support page:

After installing it, it repaired all my drivers & files, and it is now working.

This problem has come and gone on my XPS for year.   I have all the latest Dell support software and drivers.   Things had been good for about a month but with recent driver updates my Bluetooth mouse and keyboard are failing again pretty frequently.   The fix is the same as before, shutdown and unplug.
