The hard drive or SSD has failed.

If you're under warranty, the error is accompanied by a validation code.  Report both to Dell and they'll replace the drive.

If the system is out of warranty and you need help selecting a replacement for the failed drive, post the full model number (XPS L521x, 9550, etc.) and someone can assist you.
