Could be a failed fan -- that's the first thing to replace. If that doesn't solve the issue, it's the fan sensor - which is on the mainboard (and likely means it's time for a new/er system).
If replacing the fan doesn't fix the issue, the thermistor on the mainboard is bad -- in that case, you'd need to replace the main system board as the thermistor isn't separately replaceable.
