Dimension 4500 doesn't read data from USB Flash drives
My Dell Dimension 4500 doesn't read data from any USB flash (thumb) drive. I've plugged multiple flash drives into ALL of the USB ports; my computer tells me the drive(s) need to be formatted. I've verified that these flash drives are working by successfully plugging them into other PCs and being able to open, edit, and save files.
I've also plugged in other USB devices into various USB ports on my PC and they all work.

With Which OS?
If this was reinstalled did you install the chipset drivers?
Infection by specific malware can prevent usb devices from working.
insufficient power can prevent flash drives from working.
The ports on the rear of the computer support 500ma the front ports do not.

I know this is old post but may be my reply will help to others. I think you have to update firmware and usb flash drives . Here I would like to tell you common error with usb flash drives drivers.
1) Conflicts between two or more drivers caused by missing or busy resources
2) Missing drivers
3) Corrupted driver files
4) Poorly designed drivers that reduce a system’s stability and performance
5) Incompatibility with the operating system or hardware device
6) Problems in the Windows Register
