U2410 Dark Spots - Missing Order Number

I've ordered a U2410 about 11 months ago, and over time 4 dark spots have gotten progressively worse on the screen.  They look like long streaks of discoloration that stretch about 3 inches each.

The problem is that I no longer have the order number that came with this monitor, and monitors do not come with service tags.   Customer service has been stonewalling me on every attempt to get this monitor serviced or replaced.

I need help and to know if anyone else has had this issue with the U2410.
