You mean the backup disk request form?
https://support.dell.com/support/topics/global.aspx/support/dellcare/en/backupcd_form
Just make sure you're in the Dell Canada web site, not the the US one, to get the right form - look for the flag and country at the top left of the page above the Dell Logo.
