Optiplex 3060 diagnostics mode

Question

We use SCCM 2016 to image our Dell pc's.  I imaged one OptiPlex 3060 but after the image finishes the pc goes into Diagnostics mode.  I was able to unplug the pc for a few minutes and reboot into Windows 10 and update the latest bios.  I'm just curious if anyone has also run into this issue of what causes this to go into diagnostics mode after imaging.  We have the latest OptiPlex 3060 drivers on our sccm server.

The only thing I can guess is that the partition with diagnostics was the active partition rather than the OS partition.
