PERC error "device failed to start code 10"
Hello,
I'm using a perc H330 on my custom pc. The motherboard is evga x79 classified. I've 4 ssd, one is the OS (windows 10) disk connected to sata3 port, the other 3 are connected to the H330.
The problem is, when the "load storage oprom" option in the motherboard bios is set to "legacy", which allows me to see the PERC initialization screen and use CTRL+R to configure it before the motherboard bios, Windows can not start the PERC, and gives the "device failed to start (code 10)" message.
When the "load storage oprom" option is set to "none" or "uefi", the initialization screen doesn't appear, and everything works well in Windows.
However, as I want to install Windows on the raid, the option must be set to "legacy". Under this setting, not only Windows can't start the PERC, the Windows installation can not see it either (driver corrected loaded).
Help please. How to fix it?

This is an old post, but I thought I'd share my solution in case anyone ends up here. On a similar system (where a PERC card is not officially supported), I simply had to configure the RAID array using the Perc BIOS, and then turn OFF Legacy Option ROM support in the System BIOS. As soon as that was off, Windows Server installer saw the virtual disk, with no external driver needed.

@PTSDAN Original post from 2018, and your 2021 reply in 2023 has solved my issue that I just ran into! Thank you x10000 :)
- To clarify my issue; I am using a Dell PERC H330 with SATA breakout cables in a custom PC with ASROCK motherboard and Windows 11. (Board is too new to use W10 and Windows Server has issues with Intel LAN NICs on consumer motherboards, and they do not make drivers for Windows server since they do not want consumers using regular hardware I guess?)
Anyways, I was getting "Device failed to start" and "an i/o adapter hardware error has occurred" when checking device manager even after importing a VD from an old Poweredge T410 and a different RAID card, and this was my last stop before giving up... After turning off "Disable legacy option ROM" in the ASROCK bios my VD appeared in Windows immediately. Thanks again.
