Replaced Drive, Cannot Boot T610

Dell T610 running Raid 5 with 4x 500 gig drives, Windows Server 2008 R2.

Need some advice, here's the story as I understand it:

Drive failed and had yellow LED. Drive was pulled and swapped with new one while machine was running.

LED was green later and machine was shut down. 

Will no longer boot Windows, it loads the repair utility showing three options- image, memory test or command prompt. Appears to see the OS on drive D.

I have tried all the usual boot repair commands and none help. Booted with the Windows disk and it does not see an OS to repair. Any ideas?
