Thanks, but I finally got it figured out.. My problem was not the instruction page. The printer would continue to print after the instruction page.... I was closing the status window during the printing. I left it open and it popped with the "turn pages over" dialog.
Not sure if closing is what did it - but it works now!

Go to printers and faxed.
Select your printer.
Click on preferences.
Click on advanced. Check the 2-Sided printing button.
Uncheck print instruction page.
What happens?
